Freed's help center names the three note layouts its EHR push detects: subjective, objective, assessment and plan (SOAP), Single Narrative and H&P Two-Column. It then lists every field the content routes into: vitals, history of present illness (HPI), review of systems (ROS), physical exam (PE), Data, Care Plan and Follow-up.⁶ No other vendor here publishes an Elation field map. Elation's own Note Assist does not populate review of systems.²

Three published limits hold write-back at 7.5. The push needs Freed's Chrome extension kept current, and a clinician triggers it per note. The documentation never says that codes, orders, referrals or letters reach the chart. Cost scores 8.5, the highest here. Commure's Elation integration page states that "each section in the Scribe template maps to a corresponding field in Elation." A dedicated FAQ adds that Scribe "writes to SOAP note sections and can also write to Immunizations, Lab results, and Medications fields in your Elation chart."¹⁵

Every writable field here is a free text field. HPI arrives in one large free text field. Allergies, medical, surgical, family and social history, habits, current medications, review of systems, Data, Assessment and physical exam arrive as free text returned as a list. The provider opens the matching Visit Note template in Elation, and a browser extension syncs the note text in. Template names have to match between Scribe and Elation. Nothing populates a structured medication list, a coded problem list or any other discrete field. "Allergies: Penicillin" arrives as text.

Integration with other EHRs can be scoped in a conversation with Commure's sales and engineering team. Ask which Elation field each note section lands in, and whether the answer describes an application programming interface (API) call or a browser extension.

Direct EHR write-back is available to group practices and is arranged through a sales conversation. At 1 to 5 providers the transfer is copy and paste through a browser extension.

Independent evidence rests on a Department of Veterans Affairs page dated March 13, 2024.¹¹ It names Commure at the finalist stage of the Ambient Dictation track, 15 of more than 200 applicants. No ranking was published. Tali AI sits at the same stage and ties, because a screening selects an application rather than evaluating output.

Elation Note Assist, and whether you need a third party

Note Assist writes deeper into the Elation chart than anything else here. Elation documents it at field level, and Elation owns the chart.¹ Spoken vitals land in vital fields, physical findings in discrete physical exam fields and patient instructions in Care Plan. The Actions companion turns prescription, lab, imaging and referral intent into drafts a clinician approves.

Four published limits decide whether that is enough.

  • Note Assist generates no review of systems section and adds no ICD-10 codes to the visit-note draft.² A condition already on the problem list carries its existing code into the Assessment. Anything new arrives as a diagnosis name that opens a code search.
  • Note-template support is tiered. The two Complete H&P (2 column) layouts are best supported. SOAP Note and Complete H&P (1 column) are partly supported, and five other note types are not supported at all.³ A practice standardized on SOAP buys a partially supported configuration.
  • The 14 languages are transcription languages and the final visit note generates in English.⁴ Care plan instructions can separately be generated in the patient's preferred language.
  • The free allowance is ten AI notes per provider per month. Elation states the free option "will be gradually rolled out to groups of customers until it's available to everyone," so a practice evaluating Elation today may not have it.¹

Elation's help center confirms an unlimited upgrade and routes buyers to a form.¹ Its rate card publishes the add-on price. That price is never connected to the allowance or to any usage volume, so the unlimited price stays unpublished.⁵

The add-on therefore nearly doubles the cheapest seat. A vendor saying "API integration" needs Developer Platform Core at $150. Elation publishes no contract length, provider threshold or setup fee.

HealOS

Trial scores 8.5, the highest here, on twenty free sessions a month with self-serve signup and no credit card.⁷ Its Elation page describes a browser extension where staff confirm AI-prepared actions with one click. It names no Elation field, which holds write-back at 4.0, level with Sully.ai.²²

Quality controls score 3.0 on a review step stated in a single clause. No grounding method, hallucination handling or speaker separation is published anywhere on the site. The site shows an "ONC Certified" badge, for the Office of the National Coordinator for Health Information Technology (ONC). It is an image with no linked certificate, so treat it as unverified rather than disproved. Group administration is not assessable, because HealOS publishes nothing either way about seats or roles.

Nabla

Evidence scores 9.0, the highest here. Lukac and colleagues at the University of California, Los Angeles (UCLA) ran a three-arm trial in NEJM AI, with a usual-care control and non-vendor authors. The trial found that Nabla sat at minus 9.5% time-in-note.⁹ Read the methods before weighting it. The primary outcome is documentation efficiency, and accuracy was a five-point clinician self-rating. Nabla placed fourth at 90.7 in the 2025 Best in KLAS Ambient Speech segment, so no ranked win sits behind the score.¹⁰

Nabla scores 8.0 on published quality controls, joint highest here, on a fabrication-control method that splits each note into atomic facts and checks each against the transcript. Write-back scores 1.0. Elation appears on none of its published integration lists, which name Altera, athenahealth, Epic, Greenway, NextGen and Oracle.¹⁹ A practice this size can buy Nabla, so that 1.0 is a cannot-do rather than a cannot-buy.

Sully.ai

According to a single blog post dated June 24, 2026, Sully.ai asserts a cross-EHR API that writes the note back to Elation.²⁰ It names no Elation field. Write-back scores 4.0, tied with HealOS because no sentence separates them.

Trial scores 0.5 and cost 3.0. Sully.ai publishes no price, no range and no documented threshold. Its pricing page carries a demo form aimed at far larger organizations.²¹
