The need for good substance abuse EHR tools is large. In 2024, 48.4 million people aged 12 or older had a substance use disorder (SUD) in the past year.¹ About 1 in 5 people who needed treatment received it. More than two-thirds of SUD and mental health treatment facilities now keep records only in an EHR, with no paper charts.²

Which Substance Abuse EMR Platforms Should You Shortlist?

Eight platforms are ranked here. Kipu leads and Navix Health is second. BestNotes and Qualifacts tie for third, Sunwave and Netsmart tie for fifth, and Alleva and Behave Health tie for seventh. Each profile covers what the EMR documents, what it costs and the one thing to test in a demo.

Kipu

Best for: residential, detox and outpatient programs that also dispense methadone and want billing in the same system.

Kipu offers two ASAM paths. Its paid CONTINUUM link runs ASAM 3rd edition assessments, and each user needs a paid interviewer license.⁵ Kipu's 4th edition evaluation templates add three assessment forms built on the criteria ASAM released in November 2023.⁶ Utilization review plans and authorizations are tracked in the same system.⁷

Kipu has the fullest OTP workflow on this page. It handles dosing and dispensing, including tapers, split doses and liquid-dispensing pumps.⁸ Queues track take-home bottles.⁹ Treatment phases set how many take-home doses a patient can have.¹⁰ For prescribers, e-prescribing runs through DrFirst with electronic prescribing of controlled substances (EPCS).¹¹ Prescription drug monitoring program (PDMP) data is available in the same system.⁸ Kipu's in-house revenue cycle management (RCM) software covers benefits checks, utilization review and claims.⁷

Kipu is listed with the Office of the National Coordinator for Health IT (ONC). The January 2026 listing covers ten criteria, mostly privacy and security, plus data export.¹² Release forms can be revoked or set to expire.¹³ Part 2 duties appear in Kipu's business associate agreement.¹⁴ No product page describes treatment, payment and health care operations (TPO) consent or redisclosure notices. Pricing comes by quote, so ask for one along with a demo of a Part 2 disclosure.

Navix Health

Best for: residential, detox, intensive outpatient (IOP) or partial hospitalization (PHP) programs that want built-in Part 2 controls and a fast go-live.

Navix Health documents Part 2 controls more specifically than any other substance abuse EMR here. Its security page lists a consent flag "carried on every Part 2 record across the system" and an "append-only record of every disclosure event." Redisclosure warnings are "surfaced inline at the point of share."¹⁵ That means staff see the consent status before they release a record.

Navix publishes a price for a facility tier, and per-seat prices for individual practices. The Startup plan costs "$650 / month flat" for programs of any size, and larger tiers are priced by average daily census.¹⁶ Deployments take one to eight weeks.¹⁷ Navix lists ASAM and LOCUS assessments and "ASAM-aware" utilization review drafts, and NavixScribe drafts SOAP, DAP, BIRP, GIRP and SIRP notes from the session.¹⁸

Navix supports e-prescribing with EPCS and MAT lab panels.¹⁷ No methadone dosing or take-home workflow is published, so an OTP should ask to see dispensing before signing. Navix says it holds ONC certification through Drummond Group, though it publishes no listing ID.¹⁵

BestNotes

Best for: smaller outpatient and residential programs that want native ASAM 4th edition criteria at a published price.

BestNotes publishes its per-user base pricing on month-to-month terms, and add-ons such as AI notes and billing are priced separately.¹⁹

BestNotes lists ASAM 4th edition criteria as a native add-on, with CONTINUUM and CO-Triage as options.¹⁹ It also suggests when a patient should stay at a level of care or move to another.²⁰ AI notes draft SOAP, DAP and BIRP formats for individual and group sessions.²¹ E-prescribing includes EPCS for Schedule II to V medications, and the ScriptSure option connects to Bamboo Health's PMP Gateway.²¹

The only ONC listing BestNotes discloses is a 2014 Edition certification from 2016.²² Revenue cycle billing comes as a separate quote. Before you commit, ask how its release-of-information consents handle Part 2 redisclosure.

Qualifacts

Best for: opioid treatment programs and multi-site agencies that want methadone dispensing inside a certified EHR.

Qualifacts sells three EHRs to behavioral health agencies: CareLogic, Credible and InSync. It bought MethodOne, a dispensing system built for OTPs, in April 2026.²³ Its addiction treatment EHR now offers "fully integrated workflows for dosing, documentation, medication dispensing, compliance, and care coordination."²⁴ ASAM CONTINUUM and CO-Triage assessments run from inside clinical workflows.

Qualifacts publishes current ONC listings. Credible was certified in February 2026, including for e-prescribing.²⁵ Its Qualifacts iQ assistant drafts DAP, SOAP and BIRP notes and treatment plans.²⁴

Part 2 appears as a general compliance claim on the CareLogic page, with no consent or disclosure workflow described.²⁶ Pricing comes by quote, so ask which of the three EHRs fits your program's size before the demo.

Sunwave

Best for: programs that want admissions, clinical records, billing and alumni tools from one vendor, with AI-drafted assessments.

Sunwave combines Sunwave Health and Lightning Step, which took the Sunwave name in June 2026 after their 2025 merger.⁴ Existing customers stay on the platform they already use. Its in-house billing covers benefits checks, utilization review, claims and payments.²⁷

Sunwave's AI tool, SIA, drafts biopsychosocial assessments, group notes, progress notes and discharge summaries for the clinician to review before signing.²⁸ Sunwave says SIA meets "42 CFR Part 2 compliance requirements specific to SUD records," without describing the consent or disclosure mechanics. The Lightning Step platform holds an ONC listing certified in December 2023.²⁹

ASAM CONTINUUM was integrated into the Lightning Step platform in 2023.³⁰ Sunwave's own pages describe neither ASAM tools nor methadone dosing for the Sunwave Health platform. Ask which code base your program would run on.

Netsmart

Best for: multi-service behavioral health agencies that run addiction treatment and OTP services on one enterprise platform.

Netsmart covers medication treatment in three settings: office-based MAT, OTPs and residential care. It bought Netalytics, maker of the Methasoft+SMART OTP software, in October 2023.³¹ Methasoft+SMART tracks inventory and manages medication dispensing. Netsmart and ASAM announced a partnership in 2021 to integrate CONTINUUM and CO-Triage.³²

Netsmart's myAvatar holds an ONC listing certified in December 2024.³³ Netsmart describes "consent logic" that starts at the most granular level to cover federal and local requirements.³⁴

Netsmart names its billing and AI tools without describing them for addiction programs. The platform is built for agencies more than single-site programs, and pricing comes by quote. Ask for an addiction treatment reference site before you price it.

Alleva

Best for: IOP, PHP and residential programs that want in-house billing and an ambient note tool from the EMR vendor.

Alleva Billing covers charge capture, claim scrubbing, claim status and electronic remittance advice (ERA) auto-posting.³⁵ Its Echo tool transcribes sessions and structures notes in SOAP, DAP and other formats.³⁶ Alleva's own pages describe Part 2 as a requirement buyers should look for, without describing how its EMR handles Part 2 consent or disclosures.³⁷

Alleva's ONC listing appears on Drummond Group's certified products table.³⁸ It announced a System and Organization Controls (SOC) 2 Type II report in November 2025. ASAM support is described only as "ASAM-aligned treatment planning," with no EPCS, PDMP or dosing detail. Pricing comes by quote, so ask for a walkthrough of ASAM and medication workflows.

Behave Health

Best for: programs that want customer relationship management (CRM), EHR, billing and operations tools such as HR and property management from one vendor.

Rank: Tied for 7th

Behave Health "natively bundles CRM, EHR, RCM, and ERP into one system with a single login, one shared database."³⁹ It is the only platform here that markets enterprise resource planning (ERP) tools, such as human resources (HR) and property management. For Part 2, it lists "consent tracking, automated re-disclosure notices."⁴⁰

E-prescribing runs through DrFirst with EPCS support.³⁹ The EHR includes the Clinical Opiate Withdrawal Scale (COWS) and the Clinical Institute Withdrawal Assessment (CIWA) for detox. It also tracks drug tests and med pass.⁴⁰ Level-of-care assessments are listed without naming ASAM, so ask how level-of-care decisions are documented before you buy. Behave Health claims no ONC certification, and its security page lists no SOC 2 report.⁴¹

What Does 42 CFR Part 2 Require of an Addiction Treatment EMR?

42 CFR Part 2 is a Code of Federal Regulations (CFR) rule. It protects records that identify a patient as having an SUD when a federally assisted program holds them. The 2024 final rule took effect April 16, 2024, and programs had to comply by February 16, 2026.⁴² The changes an EMR has to support include:⁴³

  • Single consent. One patient consent can now cover future uses and disclosures for treatment, payment and health care operations.
  • Redisclosure. HIPAA covered entities that receive records under that consent can redisclose them under HIPAA rules.
  • Penalties and breaches. Part 2 penalties now match HIPAA's, and HIPAA breach notification rules apply to Part 2 records.
  • Segmentation. The rule states that segregating or segmenting Part 2 records is not required.
  • Counseling notes. SUD counseling notes kept apart from the record need their own consent.
  • Legal proceedings. Even under a TPO consent, records still cannot be used against the patient in legal proceedings without written consent or a court order.

Enforcement has moved to the Office for Civil Rights at the U.S. Department of Health and Human Services (HHS), and "beginning on February 16, 2026, anyone can file a Part 2 complaint."⁴⁴ One piece is still pending: compliance with the accounting-of-disclosures section is paused until HIPAA's matching rule is revised.⁴² State law can be stricter, so confirm your obligations with a healthcare attorney. How Do OTP and Office-Based MAT Needs Differ in an EMR?

An opioid treatment program dispenses methadone on site, so its EMR needs dosing, take-home and inventory tools. A clinic that prescribes buprenorphine needs EPCS and PDMP checks instead. Three federal rules shape both.

Under the 2024 update to 42 CFR Part 8, OTP patients can be eligible for take-home methadone "upon entry into treatment," based on clinical judgment.⁴⁵ Supplies are capped at 7 days in the first two weeks, 14 days through day 30 and 28 days after that. It also allows telehealth for new patients in some cases. A Drug Enforcement Administration (DEA) rule allows a first six-month supply of buprenorphine by audio-only telemedicine. The PDMP check has to be recorded in the chart.⁴⁶ It took effect December 31, 2025.⁴⁷ DEA's broader telemedicine flexibilities run through December 31, 2026.⁴⁸

Kipu, Qualifacts and Netsmart document OTP dispensing on this page. Navix, BestNotes and Behave Health document EPCS but not methadone dosing. Match the addiction treatment EMR to the medications your program actually dispenses.

Which ASAM Edition Does Your EMR Support?

ASAM CONTINUUM, the licensed ASAM assessment that EMRs integrate, is built from The ASAM Criteria, Third Edition.⁴⁹ The Fourth Edition came out in November 2023.⁶ ASAM's own Fourth Edition software is built for utilization management, so the EMRs here deliver 4th edition assessments as their own templates.

BestNotes lists native 4th edition criteria. Qualifacts and Netsmart integrate CONTINUUM.
